Follow the application checklist:
1. Confirm that you meet our driver requirements and then print the REQUIREMENTS form.
(Driver requirements page is the same as the Requirements form.)
□ Make sure you have 2 pages.
□ Print, sign and date on the bottom of page 2.
2. Print the APPLICATION form.
□ Make sure you have all 4 pages.
□ Print, sign and date the boxed section on page 1.
□ Make sure you include employment history for previous 3 years, no matter how many companies that includes and including gaps in employment.
□ Confirm that you said "yes" or "no" to all FMCSA regulation questions for each job.
□ Sign and date bottom of application on page 3 AND print, sign and date disclosure statement on page 4.
3. Print the DOT DRUG & ALCOHOL DISCLOSURE form.
□ Make sure you have all 5 pages.
□ Make sure you include employment history for previous 3 years, no matter how many companies that includes.
□ Print, sign, date and include social security # on the bottom of page 1 AND on the bottom of page 2.
4. Print the BACKGROUND CHECK form.
□ Make sure you fill out a separate form for each former employer, covering the previous 3 years, no matter how many companies that includes.
□ Print, sign, date, include social security # on the top of page 1.
5. Send all 4 completed forms (requirements, application, DOT and background check) to:

6. Allow time for processing background checks - approximately 1 week to 10 days. If you have passed underwriting and are selected by HaulBikes for employment, we will contact you to set up a start date for training. After you have been hired, you will have 1 week of paid training. Before training, owner operators will complete an exclusive contract agreeement.
